This notice explains this Scent to Signal interest register. Sofi Health Limited is responsible for the information collected here. You can contact us at hello@sofi.health.

Registering your interest does not enrol you in a study, confirm eligibility or give permission for study participation or wearable access. Those decisions will be explained separately.

what we use, and why

The questions help us understand whether Scent to Signal could suit you. Some answers concern health, including sleep conditions, medicines, pregnancy or breastfeeding, fragrance reactions and sense of smell. We ask your permission before these questions.

We ask for your name, then your email, before the screening questions. Your name, email and answers remain on the open page until you choose what to save at the end:

• S5 follow-up: we save your name, email, the answers you provided, your provisional result and the contact and health-information permissions you gave.
• Future studies only: we save your name, email and this contact choice. We do not save your screening answers or a health-related result.
• No contact: we do not save your name, email or answers. Closing or refreshing an unfinished form clears these details from the page.

We also record when you registered, the version of the questions and information you saw, and the invitation group where supplied. These records help us follow your choices correctly. We do not use this information for product marketing or advertising.

your choices

We use your permission for the screening information and contact choices described here. S5 follow-up and future-study invitations are independent choices, both unchecked initially. You can decline or withdraw without changing your sofi customer account.

The first result is generated from the answers you give. It is a provisional indication, not a clinical assessment or a final eligibility decision. You can correct an accidental answer before submitting, or contact sofi with a question.

where the register is held

This register is hosted through ChatGPT Sites using Cloudflare infrastructure and a Cloudflare D1 database. It is separate from the sofi study app. The participant form is public; stored responses are not public.

Authorised sofi administrators can access the register. The hosting services process information to operate and protect it. Hosting and support can involve processing outside the UK. Contact sofi if you need further information before providing health information.

This register does not send identifiable responses to DSM-F or automatically add them to another mailing platform. We do not sell your information.

how long we keep it

We keep S5 screening answers and the associated S5 contact record until you withdraw or we close this S5 interest register, whichever happens first. Closing the register removes the screening answers and S5-only contact records.

If you separately choose future-study invitations, your name, email and that choice remain until you withdraw them. If you later join a study, its information notice will explain any separate study records.

changing or removing your registration

After a successful registration, you receive a private management link. Keep it private. You can use it to stop S5 contact and delete your screening answers, stop future-study invitations, or remove the whole registration.

You can also email hello@sofi.health to ask to see or correct your information, withdraw permission or request deletion. We may need to confirm that the request is yours. We will explain any limits that apply.

The private link controls that registration in this register. It does not change an existing sofi customer account or subscriptions elsewhere.

protecting the form

The form uses an essential, short-lived security cookie and limited technical records to protect submissions and prevent abuse. It does not use advertising or analytics cookies. Hosting services also maintain their normal security and operational logs; we do not put names, email addresses or questionnaire answers into page URLs or application logs.

When a registration is removed, an unlinked random submission identifier may be retained to prevent a delayed retry recreating it. This marker contains no name, email, screening answer or result.
